These services offer a range of person-centered support programs designed to help individuals—particularly those who are elderly, have disabilities, or live with chronic health conditions—maintain independence by remaining in their homes or communities, rather than entering institutional settings like nursing homes.

Our HCBS operations partner directly with various state governments as approved providers for Aging Services Waivers and Developmental Disabilities Waiver Programs. These partnerships allow us to deliver compassionate, high-quality care that aligns with each state’s regulatory framework and community needs.

With over 40 years of combined leadership experience in healthcare, our executive team is uniquely equipped to replicate our successful service model across the country. Our journey began in Massachusetts in 2014, and we have since expanded into Connecticut, New Hampshire, Missouri, and Indiana. Building on this strong foundation, we are actively executing strategic growth initiatives in Ohio, North Carolina, South Dakota, Georgia, and Nevada.
